To obtain all endings of “The Nameless Hospital” in NTE, you need to collect clues and insert them in the room with the cameras on the second floor. The first floor holds clues for the first ending, the second floor for the second, and the third floor for the third. The fourth ending is more complicated, as it requires a deeper understanding of the lore of “The Nameless Hospital.”
All endings of “The Nameless Hospital” in NTE
You can obtain all four endings of “The Nameless Hospital” by entering its nightmare four times. Each time you can aim for a specific ending, gradually learning the location. In my runs I found a way to quickly collect the required clues without interacting with most puzzles. For example, these playthroughs do not include turning on the generator to supply electricity. However, at the end of the guide I will explain how to activate it.
Here’s how to get each ending of “The Nameless Hospital” in NTE:
- Ending “The Nameless Hospital” #1: collect three “Anomalous Disorder” clues.
- Ending “The Nameless Hospital” #2: collect three “Observation Log” clues.
- Ending “The Nameless Hospital” #3: collect three “Anomalous Experiment” clues.
- Ending “The Nameless Hospital” #4: collect one hidden clue and two special “Anomalous Experiment” clues.
Ending “The Nameless Hospital” #1
Getting the first ending of “The Nameless Hospital” in NTE is fairly simple—you just need to collect three “Anomalous Disorder” clues. The first one lies right on the table in front of your starting position.
Leave the room and keep moving down the corridor.
Do not enter any rooms; follow the corridor to the right.
You will reach the first‑floor lobby, where you can obtain the first‑floor key for “The Nameless Hospital” in NTE, called “Key Bundle: 1F”.
From there, go to the far end of the lobby.
You’ll find a door in the corner.
Inside you can pick up the second “Anomalous Disorder” clue in Neverness to Everness.
Now return to the lobby and continue down the corridor.
This time keep moving forward until you reach the first door after the turn.
Open the door and go inside to obtain the third “Anomalous Disorder” clue in “The Nameless Hospital.” Be careful: after you take this clue a mummy monster will appear and start a game of hide‑and‑seek. It cannot be damaged, and if it catches you you will respawn at the starting point. I didn’t find it dangerous, just irritating, because you have to backtrack to the spot where it caught you.
Don’t go back the way you came. Instead, proceed to the next room and exit it into another lobby.
Follow this lobby to the end, avoiding any rooms on the right.
Eventually you’ll reach a room at the end of the corridor with a door to its left.
Enter the new room and you’ll find a yellow screen with the fourth “Anomalous Disorder” clue. This is a bonus clue in case you missed or didn’t find the previous ones during this run of “The Nameless Hospital.”
Continue into the next room to find a room with a locked door and a cabinet to its right.
There you can find the second‑floor key called “Spare Key: 1F”.
Clear the debris, then leave the room and locate the gate leading to the second floor.
Take the stairs and you’ll immediately arrive at the main lobby of the second floor. Behind the main desk you can enter a room that contains three cameras.
Approach the cameras and insert the collected “Anomalous Disorder” clues.
After placing the clues you will receive the first ending, which concludes the first run of “The Nameless Hospital” in NTE.
Ending “The Nameless Hospital” #2
Now, when you re‑enter the nightmare of “The Nameless Hospital,” you will always start with the Staff Elevator Access Card—the key needed to activate the elevator.
To get the second ending, leave the starting room and head to the area where the monster enters.
This is the room opposite the elevator.
At the far end of this room you’ll find a small table with the first “Observation Log” clue for this run.
You can keep moving forward, exploring the location—for example, learning how to activate the generator. However, in my experience it’s much faster to head straight back to the elevator and go up to the second floor to obtain ending #2.
From the main lobby of the second floor, go to the corridor at its far end.
Before you reach the lobby, a group of monsters will appear.
After many runs I noticed that two types of monsters can appear here. Depending on which type shows up, the conditions on the second floor change. If they are were‑wolves, the rooms on the second floor will react to light: some open when the lights are on, others when it’s dark. In that case activating the generator becomes important. If they are floating creatures, the entire second floor will flood with water, and some doors will open without a key.
Either way you can still obtain ending #2. After defeating the monsters, continue to the end of the corridor.
Turn left and head to the first room after the restroom.
Its door is right behind the restroom, on the same wall.
Enter and find the second “Observation Log” clue in NTE.
Exit and keep moving along the same corridor, along the same wall, to find the room with the generator on the second floor.
Inside, on the wall, you’ll see the third and final “Observation Log” clue.
You now have all three “Observation Log” clues and are ready to finish the second run of “The Nameless Hospital” in NTE.
Return to the camera room and insert all three “Observation Log” clues.
This will complete the run of “The Nameless Hospital,” and you’ll see the cutscene for ending #2.
Ending “The Nameless Hospital” #3
To get the third ending, go straight up to the third floor using the elevator.
On the third floor you’ll see two locked grates; there’s nowhere to go. Fortunately, keys are located next to each grate. Head to the door on the right. Nearby you’ll find a clock that hides a key inside.
Open the door and move right to open a new room.
There you need to find the key hidden in the front cabinets before the timer runs out, otherwise the room will eject the character.
Take the key and exit to the door on the opposite side.
Open the door to enter a corridor. Turn right and find another room right after the turn.
This room also has a timer, and strange things happen inside. Move to the opposite side of the room to find a file with the first “Anomalous Experiment” clue in NTE.
Exit the room on the other side to enter the adjacent area with a locked door.
Destroy the debris and go outside to find another door directly opposite the one you just used.
Inside this “Nameless Hospital” room several monsters will appear.
Defeat them and explore the second “Anomalous Experiment” clue.
Return to the corridor with the elevator and open the other grate. Its key can be found in a container right next to it.
Enter and open the door on the right.
This leads you to a blocked corridor with two rooms on each side. Enter the one on the right.
Inside you’ll find the third “Anomalous Experiment” clue.
You now have three “Anomalous Experiment” clues in NTE, which is enough to finish this run of “The Nameless Hospital.”
Return to the elevator and go down to the second floor.
Enter the camera room and insert all three “Anomalous Experiment” clues.
This gives you ending #3, and you’ll have completed the “Nameless Hospital” run for the third time.
Ending “The Nameless Hospital” #4
While the first three endings are relatively easy to obtain, the final one is the hardest because it requires a lot of reading and cross‑referencing clues. Of course, I’ll try to simplify the task as much as possible with this guide.
First of all, you need either the main character or Nanalli in the squad.
Start the last run of “The Nameless Hospital,” heading straight to the elevator to reach the second floor.
From there follow the corridor after the main lobby.
Keep moving forward to the end of the corridor.
Turn left to enter the room after the restroom.
Again note that this is the room on the same wall.
This time, inside, on the right side of the wall, you’ll find a board.
It will be either hidden or visible, depending on which variation you get when monsters appear in front of the corridor.
If you get the flooding variation, the board will be visible. If you get the light‑sensitive variation, you need to interact with the switch next to the door to turn off the lights. This will reveal the board.
Now approach the board and interact with it to obtain the keys called “Key Bundle: 2F.”
Return to the start of the corridor where you’ll find a locked door. This locked room can be accessed through the room right next to it. All you need is the key you just obtained.
If you got the flooding variation, you can go straight into that room, as the key isn’t required.
Inside this room you’ll find a huge hole in the wall that leads to a previously locked room.
Here’s where things get interesting. I tried countless times to reach the room one floor above, because it holds the most important secret in the “Nameless Hospital” nightmare. Unfortunately nothing worked except a small “cheat‑method” to get there. You can do it easily by finding a raised platform that the main character can stand on.
From this platform use the character’s skill and wait for it to launch them into the air.
As soon as that happens, instantly switch to the other character and perform a double jump in mid‑air, trying to reach the floor. This can be done only with the main character or using Nanalli’s zero‑gravity mode.
Once you reach the upper room, interact with the central sealed capsule to obtain a “Puzzle” — the most important item in the “Nameless Hospital” nightmare. After obtaining the “Puzzle” there’s nothing left to do there except open the safe in NTE. Return to the second floor and use the elevator to go up to the third floor.
On the third floor, after exiting the elevator, head to the grate on the left.
Enter the room and open the door on the right.
Proceed to the room on the right side.
Inside you’ll find another “Puzzle” you need.
Return to the previous room and place both “Puzzles” on the central table.
This will trigger monster spawns and create new doors and corridors.
Use the back door to enter a small room with sealed capsules. From this room go through the wall on the right to discover a hidden room that appears after inserting the “Puzzle”.
Approach the table and read the note. You need to read it four times, as each reading changes the text along with the room.
After the fourth interaction with the note you will obtain the clue “Note V???”, which is the first clue needed to complete the fourth ending of “The Nameless Hospital” in NTE.
Now go back to the corridor where you found the second “Puzzle” and move forward to the end.
Just before the very end you’ll find a door on the right, at the start of a turn.
Interact with the files on the table to obtain “Anomalous Experiment III” — the second clue required to finish the final run of “The Nameless Hospital.”
Immediately after leaving the room, enter the room opposite.
At the end of this room you will meet Misty — an experimental object capable of transforming into other items. Ask it to follow you and exit the room into the corridor.
Turn left after exiting the room and keep following the corridor.
Turn right and move forward.
Before completing the full loop you will find a room.
Enter it and interact with the final container to turn Misty into a “Puzzle.”
Return to the central table and insert it, which will open the door leading to the last clue.
Walk the same corridor you came from and reach its end.
There you’ll see a new door on the wall. It connects two zones on the third floor.
Approach the light switch in the center of the room and perform the following sequence to open a hidden chamber:
- Press the switch twice in a row.
- Press once, wait one second.
- Press once, wait one second.
- Press once, wait one second.
- Press once, wait one second.
- Press the switch twice in a row.
Solving this switch puzzle will open a hidden chamber containing the third and final clue for the fourth ending of “The Nameless Hospital” in NTE — “Anomalous Experiment VI.”
Now return to the camera room on the second floor.
Insert the clues to complete the fourth ending of “The Nameless Hospital” in NTE.
How to activate the “Nameless Hospital” generator in NTE
To activate the “Nameless Hospital” generator in NTE, you must follow a specific order. First, turn on the middle generator at the back of the room. Then activate the one in front of it. After that aim at the generator at the far end of the room, and finally turn on the last one next to the door.
