How to Start the Quest
Activate the Mysteria Ecclesiae DLC and follow Albicus to the Sedlec Monastery to speak with Abbot Jan. This will trigger the story quest Anamnesis, where you need to find out what is ailing confessor Stepan, attendant Yaroslav, and servant Blazena.
To start the side quest ‘Maddening Pain’. Leave the infirmary and at the entrance door, listen to the dialogue between Madla and novice Benedict. How to Help Madla
